Practice Nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. The Practice Nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.

Practice Team

We have 17 reception staff to answer all your calls, queries and concerns and to book appointments. We have dedicated prescription clerks to take all your prescription requests.

We have 3 secretaries who work closely with the doctors to send your referrals to the hospitals and 7 administration staff to deal with all the other tasks involved with keeping your notes as up to date as possible.

Care Co-Ordinator

Hartlepool and Stockton Health have developed a new Care Coordinator team who will work alongside your GP to signpost you with services and benefits available locally to support you with your health and wellbeing concerns.

 

It is felt many patients may be missing out on helpful services that they qualify for and would be interested in, simply because they do not know what is available.

 

The Care Coordinators will make sure that everyone has access to the things that would improve their health and wellbeing, and will help you to take advantage of anything that they feel would make a difference to you.

Healthcare Team

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
